When someone has been killed as a result of domestic violence abuse a review should be carried out.

What happens when someone is killed as a result of domestic violence

When this happens, the law says that professionals involved in the case must review what happened so that we can identify what needs to be changed and reduce the risk of it happening again in the future.

Domestic Homicide Reviews (DHRs) were established on a statutory basis under section 9 of the Domestic Violence, Crime and Victims Act (2004) which came into effect on 13 April 2011. Under guidance issued by the Home Office, any incident of domestic violence or abuse which results in the death of the victim requires a DHR to be carried out by the local Community Safety Partnership. In Hillingdon this body is called the Safer Hillingdon Partnership.

Domestic homicide reviews are not inquiries into how the victim died or into who is responsible. The purpose of a DHR is to understand where there are lessons to be learned and make recommendations to prevent future homicides. The purpose of the multi-agency review is to ensure that agencies are responding appropriately to victims of domestic abuse and to apply any lessons learned through an action plan or recommendations. They are not inquiries into who is culpable - this is for the court or coroner to decide.

Charlotte (2015) - The Safer Hillingdon Partnership has published the Overview Report and letter from the Home Office for the domestic homicide review conducted into the death of Charlotte in 2015.
