Support at home (reablement) is a free care service offered to older people for a period of up to 6 weeks.
If a service user needs on going care this is redirected to the care managers to authorise a care package via a private agency, at this point a financial assessment will be undertaken by our in house finance team.
The aim of reablement is to maximise the service users' independence and rebuild confidence when they may have been in hospital, due to a fall or a medical condition that has changed their normal level of function. The reablement team will respond once the information is passed to them, once the service user has been discharged.
A senior carer will undertake the initial home visit and set goals and objectives with the service user. The hospital and family GP may be contacted for more information.
The reablement team consists of carers, senior carers and if needed physiotherapist or occupational therapist.
Equipment can be offered to support service users stay in their own home, including aids from the occupational therapist or the support of TeleCareline and equipment such as bed or door sensors.
Referrals are received via the hospital if service users have been admitted to hospital also from community referrals via GP's or the rapid response service.
