In-Year admission to school roll notification

All mandatory fields* need to be completed before returning this form to the Local Authority. Failure to complete all mandatory fields may lead to the young person encountering a delay in their enrolment to alternative provision.
The completion of this form will satisfy the requirement to inform the local authority of an admission to the school roll.
