We are undertaking a major system upgrade which means that our usual Portal Early Help Referral and Assessment form is unavailable.

This form has been created in order to capture your referral information. It is not an exact replication of the form that you're used to but it will serve as a means of submitting your referral details.

This form will be available between 11th August 2025 and 18th August 2025.

Helpful notes when using this form:

- An account is not required in order to submit this referral.
- There is no save functionality for this form and so you will be unable to resume your referral at a later time / date.
- Please ensure that you have your attachments ready to be uploaded on the final page of this form; there is a limit of 8 seperate attachments per referral / form.
- After submitting your form, you will be sent email confirmation including a PDF attachment of your answers.
