Your wellbeing - both physical and emotional - is a top priority. We're dedicated to supporting you in maintaining and improving your health as you transition into independence.

Whether you're looking for someone to talk to, advice on handling stress and anxiety, or resources for deeper issues, there's help available. Remember, reaching out for support is a sign of strength, and these resources are here to support you on your journey towards wellbeing. 


Your personal advisor 

Your personal advisor is your wellness ally. They can help you with lots of things, including:

  • talking about health and wellbeing - ensuring you get the support you need
  • healthcare registration - helping you to register with a GP, dentist and optician
  • setting up the NHS app - a digital tool, which is like your health passport, keeping track of your medical details and making healthcare management easier
  • completing your pathway plan health review - which includes your health details (NHS number and healthcare provider information) and ensures your health needs are always up-to-date. 

Dental and eye care 

If you're under 19 and in full-time education, or if you're receiving benefits, you're entitled to free dental and eye care. It's important to keep up with these areas of your health, and we're here to ensure you can access these services easily. 


Free prescriptions 

Qualifying for free prescriptions can ease financial stress and keep you in good health. Your personal advisor is ready to help you apply, ensuring you get the medication you need without the worry of cost. 


Transition nurse

The Hillingdon Transition Service supports 14 to 19-year-olds move from care to adulthood and can provide:

  • GP and hospital support - help with appointments and any follow-ups needed from hospital visits
  • local support and information - signposting you to local services that can support your health and wellbeing
  • health advice - offering personalised advice on staying healthy
  • links with healthcare professionals - ensuring you have a co-ordinated care approach that meets your needs. 

Mental health matters

Dealing with life's ups and downs? We've got support for that. 

  • Mental Health Pathway Lead - dedicated support for young people with complex mental health needs to navigate services and get the help they need.
  • Give Space - drama therapy and wellbeing sessions for creative expression and support.
  • P3 wellbeing service - a hub for young people to access mental health support, join activities, and find community. 
  • Mind Hillingdon - tailored support for young people experiencing mental health challenges.
  • NHS Talking Therapies - free, confidential therapy to help you through tough times.

Don't bottle it up

  • Sorted - confidential advice and support for drug and alcohol issues is just a call away.
  • ARCH - comprehensive treatment for substance misuse, because we know it's tough to tackle alone.

Love and relationships

  • Sexual health services - free and confidential advice is always available to help you make informed, safe choices. 
  • KISS - for conversations about relationships, safe sex and more.

Counselling and therapy 

  • LINK - access to local counselling services.
